What to expect on your first visit

On your first visit, we will discuss your responses on the Health Information form (see below) and any other concerns you may have. Given your situation, I will provide recommendations for additional lab work or wellness protocols. We will create a workable plan together and determine when your first follow up appointment should happen.

Policies

We understand that sometimes life gets in the way of your services with Pure Health Restored. We kindly request that you adhere to the following policies so that we may continue to offer the highest level of care to all of our clients.

We accept payment through bank transfer, credit card*, Venmo, Zelle and Paypal.
There are no refunds for services rendered or testing that has been ordered.
48 hour cancellation notice is required for all scheduled services; otherwise the client is responsible for payment in full.
